MyMNGuardian (MMG) is an application, developed by the Minnesota Judicial Branch, that allows court-appointed guardians to submit Personal Well-Being Reports (PWBR) and the corresponding Affidavit of Service electronically.
MMG is an easy electronic tool for preparing, reviewing, and filing annual reports. The new application will encourage timely and accurate filings of PWBR reports and the Affidavit of Service.
Guardians can use the application to submit reports and documents to the court electronically. Court staff will use the application to send reminders and notifications to guardians, and to accept PWBRs.

MMG is exclusively for guardian reports. Conservators are responsible for filing financial reports through MyMNConservator.
If you are a professional guardian working for an agency, you must wait until your agency administrator sets up an agency account before setting up an individual account.
Users are encouraged to register for and attend a training session to learn about the system and how to use it. The sessions are applicable statewide and are approximately 60 minutes, including time for questions. Training is available for both agency account administrators and individual guardians.

Set Up a New Agency Account
Agency Account Administrators must add professional guardian employees in order to assign them cases.
- Add employees to the agency under agency account using the employee first name, last name, and email address that will be used for log in where the employee can receive notifications
Note: employees must use the same email address to set up an individual account.
- Assign the cases that the employee will be responsible for
- Assign the appropriate employee role
Once an agency account is created, employees may set up individual MMG accounts.
Agency Account Administrator Role
The agency administrator role can:
- Assign cases to employees and manage an employee's accounts
- Complete Personal Well-Being Reports (PWBR) and Affidavits of Service for assigned cases.
- Assign the appropriate employee status
- Active for current employees
- Inactive indicates the employee is no longer working with the agency
Account Set Up
- The first time you log on to MMG, you will need to create an account.
- You MUST have an email address to create an account.
Note: if you work for an agency, you must use the email address the Agency Account Administrator used to assign your cases.
- You will create your own password for your MMG account. It is important you remember your password.
Register your case
To register your case you will need to know:
- The complete case or file number
Note: All characters including dashes and all zeros must be included when you first log into the system.
- The anniversary date of your appointment
How do I know if I need an individual account or an agency account?
You need an individual account assigned to an agency account if you are a professional guardian and employed by an agency.
If you are a professional guardian working for an agency, you must wait until your agency administrator sets up an agency account before setting up your Individual account.
Note: if you work for an agency, you must use the email address the Agency Account Administrator used to assign your cases to set up your account.
How do I know when my Personal Well-Being Report (PWBR) and Affidavit or Service are submitted?
Your materials have been successfully submitted when the Report Status displays "Accepted" on the All Cases view. The PWBR and Affidavit or Service will move from Reports Due to the Report History section.
How do I sign up to receive or change text reminders?
Sign up for, cancel, or change email and text message reminders in the Guardian Details view. You can choose to receive reminders via text, email, or both. Message and data rates may apply for text messages.
I am changing agencies, how do I transfer my account?
You cannot transfers accounts, your new agency account administrator will need to initiate the process.
How do I change my email notification preferences?
You will be automatically enrolled in email notifications when you set up your MyMNGuardian account. You may change email notification preferences in the Guardian Details view.
I am also the conservator for my case, how do I complete my inventory and annual accountings?
When I view/print my report, the entire page is not visible and/or pages are missing
This can usually be resolved within your print preview dialog box by selecting “Shrink to Fit” and/or “Shrink Oversized Pages” or similar options. For other options, consult your printer manual or help feature.
How do I determine who the ‘interested persons” are for service of my reports?
I received a notification but I already submitted my Personal Well-Being Report (PWBR)?
If you filed your PWBR but not your Affidavit of Service you will receive a notification to file the Affidavit of Service. If you believe you have completed all report filings but still received a notification please contact court administration
in the county in which the case is filed.
