IMPORTANT! There are two (2) categories of child support forms.
- "Expedited Process" (ex pro) forms are used ONLY if the County Child Support office is involved in your case.
- "District Court" forms must be used if the county is not involved in your case.

Court Forms
- The court does NOT publish forms to start child support.
- If you do not see a link to forms that fit your situation, visit your local law library to find forms, or talk to a lawyer.
- If you have a child support order, select the form packet below that deals with your issue.
Stipulations (agreements)
If all parties agree on the legal issues involving child support, you might be able to file your agreement with the court, which is called a "Stipulation and Order to _____." All parties must sign the Stipulation in front of a notary, and then one party files it with the court. The MN Judicial Branch does NOT publish sample Stipulation forms. You might find sample forms at your local law library or you could talk to an attorney.
