Proof of Insurance Submission Request Form

Please read the following instructions before submitting the Proof of Insurance Request Form: 

  1. Submitting proof of insurance documentation to the courts electronically is a two-step process.
  2. Complete the form below and submit. Do not include confidential information such as drivers’ license number, bank account, social security number, etc. in this submission.
  3. After completing the form, you will receive an email from the courts with instructions about how to submit your proof of insurance.
  4. Information submitted through this form does not change your contact information with the court. Notify Court Administration where your case is filed if you want to change your address, telephone number, or email address.
