MyMNGuardian (MMG)


Important: Starting on April 15, 2024, all guardians or their attorneys must file annual Personal Well-Being Reports and Affidavits of Service through the MyMNGuardian (MMG) online application. See Minn. Gen. R. Prac. 14.01(b)(8); Minn. Gen. R. Prac. 416

  • Your due date for filing your PWBR has not changed.  What has changed is that effective April 15 all guardians must file their report electronically using the MMG application.
  • If you have been using MMG to submit your PBWR, nothing is changing for you.  Please continue using MMG.

MyMNGuardian (MMG) is an application, developed by the Minnesota Judicial Branch, that allows a court-appointed guardian or their attorney to electronically submit Personal Well-Being Reports (PWBR) and the corresponding Affidavits of Service.

MMG is an easy electronic tool for the guardian or their attorney to prepare, review, and file annual reports. The  application encourages timely and accurate filings of PWBRs and Affidavits of Service, by sending system-generated reminders and notifications via email and/or text.

MMG is only for guardian reports.  Conservators are responsible for filing financial reports through MyMNConservator.

If you are a professional guardian working for an agency, you must wait until your agency administrator adds you as an employee, selects your role, and active status before you set up your individual account. 

 

Set Up a New Agency Account

Agency Account Administrators must add professional guardian employees in order to assign them cases.
 
  1. Add employees to the agency under agency account using the employee first name, last name, and email address that will be used for log in and where the employee can receive notifications
    Note: employees must use the same email address to set up their individual account.
  2. Assign the cases that the employee will be responsible for
  3. Assign the appropriate employee role 
  4. Select the Active empolyee status and Save.
Important: If the employee has already registered prior to being added as an employee by the agency administrator, please contact EAST to request their user account be removed:

Agency Account Administrator Role

The agency administrator role can:
  • Assign cases to employees and manage an employee's accounts
  • Complete Personal Well-Being Reports (PWBR) and Affidavits of Service for assigned cases. 
  • Assign a Company Admin
  • Add Cases
  • Assign the appropriate employee status
    • Active for a current employee
    • Inactive for an employee no longer working with the agency

Account Set Up

  1. The first time you log on to MMG, you will need to register for an account.
  2. You MUST have an email address to create an account.
    Note: if you work for an agency, you must use the email address the Agency Account Administrator used to assign your cases.
  3. You will create your own password for your MMG account.  It is important you remember your password.
  4. If you have a cell phone it is recommended that you select “Text Message” to receive your one-time passcode.
  5. Once you select “Register” you will receive an Activation email to activate your account.
  6. Once activated, login using your username and password to receive your one-time passcode.

Register your case

To register your case you will need to know:
  1. The complete court file number
    Note: All characters including dashes and all zeros must be included when you first log into the system.
Attorneys can continue to assist guardians with drafting, filing, and serving reports. It will be done through MMG (which is a free application) instead of through eFS. The guardian can still meet with their attorney (in person, over the phone or video conference) to fill out the report just as they are doing now. The report will be filled out by accessing MMG through an internet browser. This can be done using anyone’s computer, smart phone, or tablet, including the attorney’s computer.
 
If the attorney is signing the report, the attorney may type the following in the field after “Everything stated in this document is true and correct”: [Attorney’s name] on behalf of [Guardian’s name].

Future functionality will allow an attorney to have their own account for completing and filing reports on behalf of their clients. This is anticipated to be available by July 1, 2024.
 
You need an individual account if you are a guardian who does not work for a professional agency.

You need an individual account assigned to an agency account if you are a professional guardian and employed by an agency. 

If you are a professional guardian working for an agency, you must wait until your agency administrator adds you as an employee before setting up your Individual account.

Note: if you work for an agency, you must use the email address the Agency Account Administrator used to assign your cases to set up your account.

 
When you submit the report and the affidavit of service, you will receive an instant “Success” message.

The Personal Well-Being Report and Affidavit of Service will move from Reports Due to the Reports History tab.

The status on the Reports Information tab will state Submitted
In MMG, sign up for, cancel, or change email and text message reminders in the Guardian Details under your profile in the upper right hand corner. You can choose to receive reminders via text, email, or both. Message and data rates may apply for text messages.

 
You cannot transfer accounts. Your new agency account administrator will need to initiate the process.
You will be automatically enrolled in email notifications when you set up your MMG account. You may change email notification preferences in the Guardian Details under your profile.
You complete your inventory and annual reporting in MyMNConservator.
This can usually be resolved within your print preview dialog box by selecting “Shrink to Fit” and/or “Shrink Oversized Pages” or similar options. For other options, consult your printer manual or help feature.
Interested persons are defined by Minnesota Statute: Minn. Stat. § 524.5-102 - Subd. 7.
If you filed your PWBR but not your Affidavit of Service, you will receive a notification to file the Affidavit of Service. If you believe you have completed all report filings but still received a notification please,  please contact EAST:

  • Submit a MMG Business Support Form. You will receive a response within 3 business days.
MMG will allow co-guardians to sign PWBRs. When you fill out your report, one of the first questions will be “Is the co-guardian also signing this report?”

If you are unable to select the co-guardian, please contact EAST before continuing:

  • Submit a MMG Business Support Form. You will receive a response within 3 business days.
The co-guardian does not need to register for an account to co-sign the reports.

If the co-guardian has not registered their own account, they won’t receive their own reminders or be able to view reports previously submitted in MMG.
