Note: Be aware that some records are not available from court administration. This includes, but is not limited to, birth certificates, death certificates, marriage certificates, immigration records, real estate records, and Recognition of Parentage (ROP) forms.
Forms
Submit one of the following forms depending on your circumstances.
- To submit your request electronically, complete this electronic submission form.
- Copy Request Form for Agencies Use this form if you are a member of a government agency. You may eFile, email, or fax your copy request using this form. Please note that the file number for copy requests changes at the beginning of each year. If you are unsure of the new copy request court file number, please contact our office for information.
- To submit your request by mail or email, complete this copy request form. Please contact our office for our email address. To mail, complete the form and mail it to the address given on the form- along with payment- to the Copy Center.
