  • Appellate Case Forms
    • Forms and instructions to appeal to the Court of Appeals from a district court decision or governmental agency decision are available on the Clerk of the Appellate Courts page, under "Forms."
    • These forms and instructions are published by the Minnesota Judicial Branch for use in the Minnesota Court of Appeals. They include Notice of Appeal, Statement of the Case, motion forms, and packets of forms and instructions for unemployment appeals, eviction appeals, family law appeals, postconviction appeals, and administrative (certiorari) appeals. 
  • Conciliation Court Forms
    • The “Appeals” forms, with instructions, can be found at the end of the list of Conciliation Court Forms.
